How To Get A Medical Marijuana Card In Ohio

STEP 1: Schedule an appointment with a certified medical marijuana doctor who can confirm that you have one of the qualifying conditions approved by the medical board. It is recommended to bring any records that confirm your diagnosis.

List of qualifying conditions: 

  • Amyotrophic lateral sclerosis (ALS)

  • Alzheimer’s Disease

  • Cancer

  • Chronic pain

  • Crohn’s disease

  • Chronic traumatic encephalopathy (CTE)

  • Epilepsy (seizures)

  • Fibromyalgia

  • Glaucoma

  • HIV/AIDS

  • Hepatitis C

  • Multiple Sclerosis

  • Parkinson’s disease

  • Posttraumatic stress disorder (PTSD

  • Sickle cell anemia

  • Spinal cord injury

  • Tourette syndrome

  • Traumatic brain Injury (TBI) 

  • Ulcerative colitis

STEP 2: Meet with the certified medical marijuana doctor and discuss your condition(s) and treatment history. If the doctor can confirm your diagnosis, then he/she will register you with the Patient registry. In order to do this, you must have and bring with you one of the following;

  • a valid Ohio’s driver’s license

  • a valid Ohio identification card issued by the Ohio bureau of motor vehicles

  • a valid United States passport.

If a caregiver is required, they too must be registered and will need bring the above-mentioned documents.

STEP 3: Once the certified medical marijuana doctor has entered you and/or the caregiver into the registry, you will receive an email from the Board of Pharmacy, addressed from “no-reply-pmpaware”. You will need to submit a $50 registration fee to the board of pharmacy. After this, you will create an account in the patient registry and will be able to download your official Ohio medical marijuana card.

STEP 4: Find a dispensary. You or the caregiver will need to show the dispensary an active medical Marijuana card and your associated government-issued ID
